29 CFR  1910.134(c):The employer did not develop and implement a written respiratory protection program with required worksite-specific procedures and elements for required respirator use:   At a worksite located at 12505 Northeast 41st Street, Kansas City, Missouri. 3M half face respirators were being used in the paint area and there was no written respiratory protection program.  Employees were not provided a medical evaluation prior to being required to use the 3M half face  respirators.

29 CFR  1910.178(l)(1): The employer did not ensure that each power industrial truck operator was competent to operate a power industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in paragraph (1).  At a worksite located at 12505 Northeast 41st Street, Kansas City, Missouri.  Employees were exposed to serious caught-in, crushing and or struck-by injuries, when operating, forklifts in the warehouse, including but not limited to the Toyota brand forklift model # 7FGCU32, Serial #nn 65223. Forklift operators were found to have not completed a competent training program.

Employees were exposed to serious struck by injuries from the Toyota Brand  Forklift, serial number 65223, the anti-slip foot brake cover was found to have been missing.

Employees were exposed to  amputation hazards  from the RDR wire machine. The pulleys that color and load the wire/cable on to spools were not covered or guarded to prevent rotating parts  from contacting the operator.

29 CFR  1910.303(g)(1)(i): Work space was less than 30 inches wide in front of electric equipment operating at 600 volts, nominal, or less:  At a worksite located at 12505 Northeast 41st Street, Kansas City, Missouri. Employees were exposed to fire and electrical hazards in the warehouse due to failure to maintain adequate working space in front of  the electrical circuit breaker boxes on the west wall of the warehouse. The electrical circuit breaker boxes were blocked by a collection of miscellaneous supplies.
