29 CFR 1926.351(b)(4): When a cable, other than the cable lead referred to in paragraph (b)(2) of this section, became worn to the extent of exposing bare conductors, the portion thus exposed was not protected by means of rubber and friction tape or other equivalent insulation:  On or about May 3, 2023, an employee was using a Miller Trailblazer 325 welder with a cable exposing bare conductors, exposing the employees to an electrical shock hazard.
