29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  (a ) On or about May 3, 2023, employees were exposed to machine hazards associated with moving parts when performing service and maintenance such as but not limited set-up and and the changing of grinding wheels on equipment such as but not limited to Blanchard Model 3260 60" Rotary surface Grinder, Mattison Surface Grinder Serial Number 17764, Mattison Surface Grinder Serial Number 2335, Mattison Model 60 Surface Grinder Serial Number 60-33, Mattison Model 84 Surface Grinder Serial Number 84-42, Mattison Model l60E Rotary Surface Grinder Serial Number 48-571, Blanchard Model 3260 HD Rotary Surface Grinder Serial Number 14421 and Mattison Double Housing Planer Surface Grinder Serial Number PG-24.     (b) On or about May 3, 2023, employees were exposed to machine hazards associated with moving parts when inserts are changed on equipment such as but not limited to Gooda Type VM-1625, ,CNC Plate Milling Machine Serial #200455 and Good Type VM-1200NC, CNC Duplex  Head Milling Machine Serial #190405.     The employer failed to implement energy control application steps such as LOTO devices were not applied to the electrical disconnect was not performed [per the 1910.147(d)(4) requirements]. As a result, the remaining applicable energy control elements, involving dissipation machine isolation [(d)(5)(2)], and verification of isolations [(d)(6)], were not  implemented to protect employees from machine servicing hazards.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
