29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided:  a). This violation occurred on or about April 22, 2023, when the employer did not ensure that personal protective equipment was provided and used by employees, who worked on or near energized electrical equipment and circuits, to protect them from arc flash, electrical shock and burns.

29 CFR 1926.416(a)(1): When an employee was not protected against electric shock by de-energizing the circuit and grounding it or by guarding it effectively by insulation or other means, the employer permitted an employee to work in proximity to a part of an electric power circuit that the employee could contact the electric power circuit in the course of work:  a)  On or about April 22, 2023, when the employer permitted employees to work in proximity to electric power circuits who were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means.

29 CFR  1926.417(b): Equipment or circuits that are deenergized shall be rendered inoperative and shall have tags attached at all points where such equipment or circuits can be energized.   a)  On or about April 22, 2023, when the equipment or circuits that were deenergized were not rendered inoperative/lockout and tagout all points where such equipment or circuits could be energized.
