29 CFR  1910.147(d)(4)(ii):Lockout devices utilized were not affixed in a manner that held the energy isolating device in a safe or off position:  (a) 1270 Lincoln Road, Birdsboro, PA, Baler main electrical disconnect - On or about May 4, 2023 employee lockout devices were placed on the main electrical disconnect but were not securely locked with the key removed from the lock.   No abatement certification or documentation required.

29 CFR  1910.147(c)(6)(ii):The employer did not certify that periodic inspections of the energy control procedures had been performed:  (a) 1270 Lincoln Road, Birdsboro, PA - On or about May 4, 2023, employees who were conducting servicing or maintenance activities on equipment throughout the facility were not certified as having received a periodic inspection of performing lockout/tagout.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
