29 CFR 1926.21(b)(2):The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.  a) Gate house located on  Reunion Way, Hoschton,  GA 30548: On or about 5-3-2023, The employer failed to instruct employees in the recognition and avoidance of unsafe conditions in the workplace to include fall hazards of 13-feet, 11-inches to the lower level while performing demolition work on a roof. Employees were exposed to fall hazards using extension ladders at improper angles and not extending side rails at least 3-feet past the landing areas to access upper levels. Employees were exposed to struck-by hazards while performing demolition operations and  working below a structure which was not secured as eye protection and head protection was not donned prior to the start of work.
