The employer did not ensure that two lead craft foremen (from Turner Industries Group and Universal Plant Services) attached their locks to the multiple padlock hasp of the MOV-2514 breaker per the electrical energy isolation procedure (S-104-Procedure for Locking Out Electricity Driven or Controlled Equipment).

A chain and lock on the actuator handwheel of the MOV-2516 (Motorized Operated Valve) were removed without following energy isolation procedures.  b.)	Electrical energy isolations for MOV-2514 and MOV-2516 were not shown to craft leads and were not locked.  c.)	Permit verifiers did not review and ensure energy isolation list (EIL) and energy isolation drawing were matched and electrical energy isolations for MOV-2514 and MOV-2516 were locked.
