29 CFR  1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level, the concrete floor,  was not  protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  340 West Brookfield Rd., New Braintree, MA: On or about May 8, 2023, an employee was working while standing on pipes which were more than 12 feet above the ground below and was not was not  protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19 (d) (1).

29 CFR  1926.501(b)(4)(ii):Each employee on a walking/working surface was not  protected from tripping in or stepping into or through holes  (including skylights) by covers:  340 West Brookfield Rd., New Braintree, MA: On or about May 8, 2023, 2 employees were not protected from falling through a floor hole in the walking working surface where the metal floor grates ( 99" long, 29" wide >44" deep) had been removed.

29 CFR  1926.1053(b)(1):When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  340 West Brookfield Rd., New Braintree, MA: On or about May 8, 2023, an employee was using a 12 foot A frame ladder to reach his work area which was 12 feet, 11 inches  above the concrete floor that the ladder was placed, so  the ladder side rails did not extend any distance above the upper landing surface to which the ladder was used to gain access.

29 CFR  1926.503(b)(1):The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training:  340 West Brookfield Rd., New Braintree, MA: On or about May 8, 2023, the employer had not prepared a written certification record of fall protection training which included the name or other identity of the person trained,  the signature of the person trained, date(s) the training was completed and the signature of the person who conducted the training.
