29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) In the machine shop area: On or about May 09, 2023, and at times prior thereto, employees were exposed to flying chips while using the Cincinnati Bickford Drilling machine in that it was not provided with a chip guard.

29 CFR  1904.41(a)(2): The employer's establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and was establishment is classified in an industry listed in appendix A to subpart E of this part and did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee by March 02, 2023.   a) Facility located at 1500 Hwy 22 W: On or about May 09, 2023, and at times prior thereto, the employer failed to electronically submit information from their OSHA Form 300A, Summary of Work-Related Injuries and Illnesses or equivalent form, for calendar year 2022 on or before March 02, 2023.  The establishment employed 45 employees during calendar year 2022 and was classified under NAICS code 493110.
