29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  (a) Worksite:  On or about 5/10/2023, employees were not protected from falling while working on a residential roof, approximately 18 feet 10 inches above the ground level.

29 CFR  1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or were not designed, installed, and used as follows.   (a) Worksite:  On or about 5/10/2023, employees were exposed to fall hazards while connecting two (2) rope lanyards to one hinged roof anchor that was designed to support only one person at a time, approximately 18 feet above the lower level.
