Roofers were removing shingles and installing new metal roofing materials on a 12/12 pitch roof that varied in height from 9-16 feet. The roofers were wearing climbing harnesses with climbing ropes tied to anchor points, not attached to the harnesses. The employees were not utilizing compliant fall protection which exposed them to falls of over 16 feet.
