29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).   a)  On May 10, 2023, employees were exposed to falls ranging from 9 feet 7 inch - 11 feet 8 inch while performing framing activities on a second story multi-family dwelling. 29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) On May 10, 2023, employees accessed second story using portable ladder with side rails positioned approximately 10 inches below elevated working surface and were exposed to falls from approximately 10 feet to ground surface below.  Lost River Roofing Construction was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1562505, citation number 1, item number 2a and was affirmed as a final order on April 26, 2022, with respect to a workplace located at 307 Miners Way, Mackay, Idaho 83251.
