29 CFR  1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  a) Jobsite located at 1701 N. Front Street, Philadelphia, PA. - On or about May 11, 2023, the employer did not ensure that employees performing plumbing activities in a trench measuring approximately 14 feet 5 inches in length ,11 feet in width, and 10 feet in depth, were protected from cave-ins by the use of an adequate protective system.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  a) Jobsite: 2100-2200 Parish St. Philadelphia, PA. - The employer did not ensure the employees working inside an approximate 10 foot deep trench were provided with safe access by extending the side rails of the extension ladder inside the trench at least 3 feet above the upper landing surface.  Conditions were observed on or about February 15, 2023.
