29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Hildebrant Home Exteriors, CNY Veterinary Services at Animal Kingdom, 5252 NYS-31, Clay, NY 13041 - North End of the building. On or about May 11, 2023, workers were performing roofing activities and used an aluminum extension ladder to access and egress the roof 13 feet eave to ground. The ladder side rails did not extend at least 3 feet above the upper landing. The employer did not ensure the ladder rails extended at least 3 feet above the upper landing.

29 CFR  1926.501(b)(11): Steep roofs. Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe-boards, safety net systems, or personal fall arrest systems.  a) Hilderbrant Home Exteriors, CNY Veterinary Services at Animal Kingdom, 5252 NYS-31, Clay, NY 13041 - On the roof.  On or about May 11, 2023, workers were removing and replacing the roof. The employer did not ensure that each employee was protected from fall hazards as required. Workers were exposed to falls of approximately 13 feet eave to ground during re-roofing operations without fall protection on a 6:12 pitched roof. Workers were not connected to anchor points.
