Shop Building - On or about May 10, 2023, the employer did not ensure that the shop building was kept in a clean orderly fashion where there were areas that had old truck parts, welding cables and hoses, electrical cords, and other debris on the floor which created a tripping hazard to employees using the shop.

Shop Building - On or about May 10, 2023, the depth of the working space in front of the electrical circuit breaker panel was less than the required 36 inches indicated in Table S-1, where there was items stored on the floor in front of the panel blocking access to it.
