29 CFR  1910.124(b)(4)(ii):Mechanical ventilation did not conform to NFPA 34, Standard for Dip Tanks Containing Flammable or Combustible Liquids:  a.)	Channellock, Inc. Plant #2, "Big Blue" dip tank area, on or about May 11th, 2023 and times prior thereto:  The mechanical ventilation system associated with the "Big Blue" dip tank area was not cleaned frequently enough to ensure excess residues didn't accumulate within the ventilation system/duct work.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1910.124(j)(1):Hoods and ductwork of the ventilation system were not inspected for corrosion, excess buildup of material, or damage:  a.)	Channellock, Inc. Plant #2, "Big Blue" dip tank area, on or about May 11th, 2023 and times prior thereto:  The hoods and ductwork associated with the "Big Blue" dip tank area were not inspected regularly enough in that, noticeable accumulations of residues/materials were built up at the entry point of the duct work associated with the ventilation system of the dip tanks.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Plant #2, "Big Blue" dip tank area, on or about May 11th, 2023 and times prior thereto:  The employer did not ensure that the air flow was adequate for the ventilation associated with the "Big Blue" ventilation system in that the employer did not conduct annual or quarterly ventilation tests to ensure the airflow was adequate.

29 CFR  1910.151(c):Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a.)	Channellock, Inc. Plant #2, "Big Blue" dip tank area, on or about May 11th, 2023 and times prior thereto:  Employees worked with and around corrosive chemicals including, but not limited to, JC 701C Alkaline cleaner during additions of the product to the "Big Blue" dip tanks.  The emergency eyewash/shower station (plumbed) was not in compliance with ANSI/ISEA 358.1 for Emergency Eyewash and Shower Equipment in that:     1.    Consistent weekly visual inspections of the eyewash station and shower were not conducted.  2.    The emergency eyewash unit was not activated weekly to verify proper operation.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
