On or about May 9, 2023, at the above addressed jobsite, employees using a pneumatic nail gun did not wear safety glasses, thereby exposing the employees to the hazards associated with eye injuries.

On or about May 9, 2022, at the above addressed jobsite,  employees performing residential carpentry operations, approximately  thirty (30) feet  above the ground, were not protected from falls, thereby exposing the employees to fall hazards.

On or about May 9, 2023, at the above addressed jobsite, the stairways from the ground to the third floor of the house was not provided with handrails, thereby exposing an employees to fall hazards.
