29 CFR  1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) Shop Floor, Front of the Bay: On or about April 18, 2023 and times prior, an employee operated the "Poreba Lathe" to machine parts. The employer did not ensure that the lathe was equipped with a chuck guard to protect the employee from the struck-by hazard of the rotating part.

29 CFR  1910.215(a)(4):  Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  (a) Shop Floor: On or about May 1, 2023 an employee used The Hisey-Wolf Machine Co. pedestal grinder to sharpen drill bits. The work rest on the grinding machinery was not properly adjusted, creating an opening between the wheel in the work rest that exceeded one-eighth inch, exposing the employee to a caught in hazard.

29 CFR  1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  (a) Shop Floor: On or about May 1, 2023 an employee used The Hisey-Wolf Machine Co. pedestal grinder to sharpen drill bits. The tongue guard on the grinding machinery was not properly adjusted, creating an opening between the wheel in the tongue guard that exceeded one-fourth inch, exposing the employee to a struck-by hazard.
