29 CFR  1910.147(c)(4)(ii):The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to (c)(4)(ii)(A) through (c)(4)(ii)(D) of this section.  At the establishment located at 2645 Industrial Parkway South, Marinette, Wisconsin; the employer did not ensure that the equipment specific energy control procedures clearly and specifically outlined the rules and techniques necessary to control hazardous energy for employees who perform servicing and/or maintenance operations, such as but not limited to disc grinding wheel changes and preventative maintenance, exposing employees to caught in hazards in the following instances:  a) The energy control procedure for the Side #5 Grinder (Model SDG 5/6-30, SN 748-2) did not identify all sources of potentially hazardous energy, such as but not limited to kinetic energy, the magnitude of each source of hazardous energy, such as but not limited to hydraulic energy, methods to dissipate or restrain stored energy, such as but not limited to pneumatic energy, and specific requirements for testing to determine and verify the effectiveness of lockout devices for all hazardous energies, such as but not limited to hydraulic energy, kinetic energy, and pneumatic energy.   b) The energy control procedure for the Side #4 Grinder (Model SDG6-30, SN 748-3) did not identify all sources of potentially hazardous energy, such as but not limited to kinetic energy and pneumatic energy, methods to dissipate or restrain stored energy, such as but not limited to pneumatic energy, and specific requirements for testing to determine and verify the effectiveness of lockout devices for all hazardous energies, such as but not limited to kinetic energy and pneumatic energy.

29 CFR 1910.147(f)(1)(ii): Testing or positioning of machines, equipment or components thereof. In situations in which lockout or tagout devices must be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment or component thereof, the specified sequence of actions in (f)(1)(i) through (f)(1)(v) of this section shall be followed, including removing employees from the machine or equipment area in accordance with paragraph (e)(2) of this section.   At the establishment located at 2645 Industrial Parkway South, Marinette, Wisconsin; the employer did not ensure that employees were removed from the machine or equipment in accordance with paragraph (e)(2) of this section when testing or positioning of machines, equipment or components, exposing employees to caught in hazards in the following instances:  a) On or about May 4, 2023, while an employee was performing testing or positioning of the Side #5 Grinder (Model SDG 5/6-30, SN 748-2) during setup, the employee was not removed from the equipment nor were alternative protections equivalent to lockout provided to prevent employee exposure to the caught in hazard of the infeed belt.  b) On or about and at times prior to September 6, 2023, while employees were performing testing or positioning of the Side #5 Grinder (Model SDG 5/6-30, SN 748-2) and Side #4 Grinder (Model SDG6-30, SN 748-3) during setup, the employees were not removed from the equipment nor were alternative protections equivalent to lockout provided to prevent employee exposure to the caught in hazard of the disc grinding wheels.
