29 CFR 1910.178(o)(1): Load(s) on powered industrial truck(s) were not stable or safely arranged:  a) On or about May 10, 2023, at the billet saw operation cast house, the employer allowed employees to move a bundle of four 24-foot long aluminum logs, which were not secured to the forklift, weighing 5,690.88 LBS with a class 5 Hyster powered industrial truck (forklift Model H60XT Serial Number A3080V09815U), exposing employees to crush and struck by hazards.

29 CFR  1910.178(o)(2):Load(s) were being handled which exceeded the rated capacity of the industrial truck(s).  a) On or about May 10, 2023, at the billet saw operation cast house, the employer allowed employees to move a bundle of four 24-foot long aluminum logs, which were not secured to the forklift, weighing 5,690.88 LBS with a class 5 Hyster powered industrial truck (forklift Model H60XT Serial Number A3080V09815U). The bundle of aluminum logs exceeded the rated capacity of 5650 LBS, exposing employees to crush and struck-by hazards.
