29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before an employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a) Car Wash Bay - On or about June 1, 2023, the employer did not establish a program that included energy control procedures for employees to follow, employee training, and periodic inspections to protect employees from injury caused by the unexpected energization, start up or release of stored energy when performing set up and servicing/maintenance of equipment, including but not limited to the chain drive components on the car wash equipment.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the following means:  a) Service Utility Area of Carwash Bay - On or about May 11, 2023, the employer did not ensure that the electrical panel junction box powering the carwash equipment had a cover on and effectively closed.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR  1910.303(f)(1): Each disconnecting means required by this subpart for motors and appliances shall be legibly marked to indicate its purpose, unless located and arranged so the purpose is evident:  a) Service Utility Area of Carwash Bay - On May 11, 2023, the employer did not ensure that the electrical switch panel board was identified on the face or inside of the panel door.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
