OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  On or about May 15, 2023  a. Upper level of the Vertical Reciprocating Conveyor(VRC):  Employees were exposed to a fall hazard of over 10 feet when the VRC carriage was not at the landing, creating an open whole to the basement level below.   b. Lower level of the Vertical Reciprocating Conveyor (VRC): Employees exposed to a stuck by and or crushing hazard when the VRC carriage traveled down to the lower level.

29 CFR  1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  Bakery: On or about May 15, 2023 , an eyewash capable of providing fifteen minutes of flow, was not provided where corrosives such as Accu-Chem Liquid Chlorinated Product and Dish Machine Detergent was used in the dishwasher machine and bleach was used to clean floors.
