29 CFR 1926.502(h)(1): The employer did not did not designate a competent person to monitor the safety of other employees and/or did not ensure that the safety monitor complied with the requirements listed in (h)(1)(i) through (h)(1)(v) of this section:  a)  At the worksite, 3131 Erie Blvd E, Syracuse, NY 13214.  On or about May 16, 2023, employees were engaged in roofing activities on a 4/12 pitch roof, approximately 25 feet above a lower level.  The employer failed to ensure the employees were protected from falling by use of guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.
