29 CFR 1926.453(b)(2)(v): A lanyard was not attached to the boom or basket when working from an aerial lift:  On or about 05/12/2023, the employer did not ensure that employees working in the baskets of aerial lifts were connected into the boom or basket by use of a fall protection lanyard.   A.  An employee was working in the basket of an aerial lift (Genie S-40, ID number 7825) without a fall protection lanyard.  The employee was exposed to the hazard of falling approximately fifteen (15) feet.  B.  An employee was working in the basket of an aerial lift (JLG 450AJ) without a fall protection lanyard.  The employee was exposed to the hazard of falling approximately fifteen (15) feet.

29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who was on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems:  On or about 05/10/2023, the employer did not ensure that employees were protected from fall hazards while performing structural steel construction.  Employees were exposed to the hazard of falling up to approximately twenty (20) feet when they were installing structural steel roof panels and no fall protection was being used.
