29 CFR  1926.451(b)(1):29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports:  On or about May 17, 2023, employees were exposed to a fall of approximately 20 feet while performing masonry activities on a residential home located at 3120 Old Brainard, Pepper Pike, Ohio 44124.  The employees were working from a supported scaffold and the employer did not ensure the working level was fully planked with no more than a one inch gap.

On or about May 17, 2023, employees were exposed to a fall of approximately 20 feet while performing masonry activities on a residential home located at 3120 Old Brainard, Pepper Pike, Ohio 44124.  The employees were working from a supported scaffold with a base to width ratio of 4 to 1 and the employer did not ensure it was restrained from tipping.
