29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   (a) 708 South Montgomery St. Starkville, MS - On or about May 16, 2023, an employee was exposed to a fall of  12 feet or more while working on a roof without fall protection or a safety net system.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a ridgid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the the ladder:  (a) 708 S Montgomery St. Starkville, MS  - On or about May 16, 2023, employees were exposed to a fall hazard while climbing the 12 -24 ft ladder to access the roof that did not extend 3 feet above the landing.

29 CFR  1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  (a) Jobsite - On or about May 16, 2023, employees were not trained to recognize and avoid unsafe conditions in that they were working from heights on a roof without fall protection, exposed to a fall hazard.
