29 CFR 1926.451(g)(2):The employer did not provide fall protection for employees erecting or dismantling supported scaffolds: Site- An employee dismantling a work platform on a tubular welded frame scaffold was exposed to a fall of approximately 40 feet where fall protection was not utilized. The employer's competent person had determined that the use of a personal fall arrest system was feasible to use while the employee was dismantling the guardrail system and the wood planking for the work platform.

29 CFR 1926.451(h)(2)(ii): A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below. Site- An employee working underneath a tubular welded frame scaffold was not protected from an overhead struck by hazard where multiple work platforms on the scaffold were covered with concrete block debris and did not have toe boards.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. Site- An employee assigned to dismantle guard rails and work platforms on a tubular welded frame scaffold were not provided with training to recognize hazards including, but not limited to, fall hazards and the means to minimize the hazards.
