29 CFR 1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).  a) Jobsite located at 1613 Dyre Street, Philadelphia, PA. - The employer did not ensure that the supported tubular welded frame scaffold used by employees to conduct chimney repair operations from the roof of a residential building was fully planked between the uprights, thereby exposing employees to slip and fall hazards.  Condition observed on or about May 19, 2023.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) Jobsite located at 1613 Dyre Street, Philadelphia, PA. - The employer did not ensure that employees conducting chimney repair activities from a supported tubular welded frame scaffold erected on the roof of a residential building, approximately 40' above the ground level, were protected from fall hazards by the use of a fall protection system.  Condition observed on or about May 19, 2023.

Condition observed on or about May 19, 2023.
