29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  Worksite: On or about May 23, 2023, the employer did not ensure that the employees who were required to use pneumatic nail guns while installing roofing shingles were provided with eye protection.

Worksite: On or about May 23, 2023, the employer did not ensure that the employees who were required to work from the steep roof of a commercial building, approximately 8 feet 10 inches from the ground, were provided with and required to use fall protection.

Worksite: On or about May 23, 2023, the employer did not ensure that the employees who were required to gain access to the roof using an extension ladder at a height of approximately 8 feet 10 inches was extended three feet above the landing surface.
