29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)   A.           On or about 05/23/2023, at the site of 39-16 219th Street. Bayside, NY    Employees working on the fabricated frame scaffolds were exposed to the hazard of falling approximately 15 ft. to the ground level. The working platforms were installed with only one scaffold plank,  creating a gap of 18 inches.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs g(1)(i) through g(1)(vi) of this section, each employee working on a scaffold more than 10 ft. above a lower level was not protected from falling to that lower level by the use of guardrails or personal fall arrest systems:  A.           On or about 05/23/23, scaffolds at 39-16 219th Street. Bayside, NY    Employees working from an unprotected tubular welded frame scaffolds were exposed to a fall hazard of approximately 15 ft. while installing brick to face of 2-story house. The scaffolds that the employees were working on were not installed with guardrails or personal fall arrest systems.
