29 CFR 1910.30(a)(1): The employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart before any employee is exposed to a fall hazard. And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.  On and before 4/25/2023, the employer failed to ensure employees who were required to use personal fall protection systems while working 55 feet above a lower level were properly trained.  NOTE: Each employee must be trained by a qualified person and the training must include the nature of the fall hazards; the procedures to be followed to minimize those hazards; the correct procedures for installing, inspecting, operating, maintaining, and disassembling the personal fall protection systems that the employee uses; and the correct use of the personal fall protection systems, including, but not limited to, proper hook-up, anchoring, and tie-off techniques, and methods of equipment inspection and storage, as specified by the manufacturer.
