29 CFR  1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone:   On or about May 24, 2023, and at times prior, at a worksite located at 5800 Blue Lagoon Drive, Miami, FL 33126,  employees were exposed to a 140-foot fall hazard while performing roofing work on a flat roof with warning lines erected less than 6 feet from the roof edge and no safety monitor.

29 CFR  1926.502(f)(1)(i): When no mechanical equipment was being used the warning line was not erected at least 6 feet (1.8m) from the roof edge:    On or about May 24, 2023, and at times prior, at a worksite located at 5800 Blue Lagoon Drive, Miami, FL 33126,  a warning line system was placed at a distance of 4 feet from the edge of a roof, exposing employees to a 140-foot  fall hazard.
