29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary. a) On or about May 24, 2023, on the northeast side of the site, employees were performing masonry work on the outside of a structure while utilizing a scaffold with gaps in-between planks larger than one inch wide at heights upwards of thirteen (13) feet.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects. For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall. a) On or about May 24, 2023, on the northeast side of the site, employees were performing masonry work while utilizing scaffolding that had no means of protecting employees from falling objects, tools or other materials.
