29 CFR 1926.652(g)(1)(iii): Employees shall be protected from the hazard of cave-ins when entering or exiting the areas protected by shields. a) Trench, Intersection of Havant Way and King's Chapel Road: On or about May 23, 2023, the employer exposed employees to cave-in/engulfment hazards in that the means of egress was located outside the trench box while working inside an excavation measured approximately 14-feet deep by 12-feet by 24-feet long.

29 CFR 1926.1053(b)(16):Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired. a) Trench, East side of the trench: On or about May 23, 2023, and at times prior, the employer exposed employees to fall hazards of up to 14-feet an extension ladder with damaged and missing rungs and rung locks was used to access/egress, in that a damage-free ladder was not provided to employees engaged in framing activities.

After that time, that data was not made available to the CSHO when requested. a) Excavation located at the intersection of Havant Way and Kings Chapel Road, Perry, GA, 31069, Avion Landing Subdivision: On or about 6/06/2023, the employer failed to provide the tabulated data for the trench box being used on site as strut locking pins missing, sidewall damage, and a lack of backfill to prevent lateral movement were observed while employees worked within the trench box provided.
