29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): (a) 384 Pottsville St Clair Hwy, Pottsville, PA 17901: Employees were exposed to shock hazards by working with electrical cords that were not visually inspected before use on a Raymond 6210 Pallet Stacker Truck, Serial # 621-20-15452, identification number 73AR, on or about May 25, 2023. (b) 384 Pottsville St Clair Hwy, Pottsville, PA 17901: Employees were exposed to shock hazards by working with electrical cords that were not visually inspected before use on a Raymond 6210 Pallet Stacker Truck, identification number 73BR, on or about May 25, 2023.
