29 CFR  1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a) New residential construction located at The Henderson Ridge Subdivision at 1810 Jake's Cove, Lot #28, Loganville, GA 30052, Front roof: On or about 05/25/2023, and at times prior, the employer exposed an employee to struck-by hazards while operating a pneumatic nail gun, in that protective eyewear was not provided to employees engaged in siding installation while operating this tool at 120 PSI.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) New residential construction located at The Henderson Ridge Subdivision at 1810 Jake's Cove, Lot #28, Loganville, GA 30052, Front roof: On or about 05/25/2023, and at times prior, the employer exposed an employee to fall hazards on a 12/12 pitched roof with an eave height of approximately 13-feet, 7-inches to the lower ground level, in that a fall protection system was not provided while employees were engaged in installing siding on a residential structure.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) New residential construction located at The Henderson Ridge Subdivision at 1810 Jake's Cove, Lot #28, Loganville, GA 30052, Front roof: On or about 05/25/2023, and at times prior, the employer exposed an employee to fall hazards of an eave height of approximately 13-feet, 7-inches to the lower level, in that the ladder side rails of the extension ladder did not extend at least 3-feet past the roof landing to allow for safe access on and off the roof.
