Production Floor, East Wall Floor Hole:  On or about May 24, 2023 and at times prior employees were exposed to tripping hazards while walking and working in and around the unprotected 7 inch wide, 11 inch long, 10 inch deep floor hole.  b)  West Paw, Inc. Production Floor, East Wall Floor Drain:  On or about May 24, 2023 and at times prior employees were exposed to tripping hazards while walking and working in and around the unprotected 4.5 inch round floor drain.

South Wall Scale:  On or about May 24, 2021 and at times prior employees were exposed to electric shock and fire hazards while using an orange electrical extension cord as a permanent means of wiring to power a scale for a cellophane wrapper.
