On or about May 30, 2023, an employee was ascending and descending the roof using a ladder that did not extend three feet above the upper landing surface. The employee was exposed to fall hazards of approximately 18 feet to the ground.

On or about May 30, 2023, an HVAC Service Technician was performing HVAC unit maintenance on the roof. The employer did not ensure the employee was protected from falling by guardrail systems, safety net systems, or personal fall arrest system. The employee was exposed to fall hazards of approximately 18 feet to the ground level.

On or about May 30, 2023, an HVAC Service Technician was performing HVAC unit maintenance on the roof. The employer did not ensure the worker was trained in the nature of the fall hazards in the work area and how to recognize them. The employee was not wearing fall protection and exposed to fall hazards of approximately 18 feet to the ground level.   b) Heron Mechanical at Little Caesars, 420 7th North, Liverpool, NY - On the roof. On or about May 30, 2023, an HVAC Service Technician ascended and descended the roof while performing HVAC unit maintenance using a ladder that did not extend 3 feet above the landing. The employer did not ensure the worker was trained in the nature of the fall hazards in the work area and how to recognize them. The employee was not wearing fall protection and exposed to fall hazards of approximately 18 feet to the ground level.
