29 CFR  1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  On or about October 17, 2023, the employer failed to submit documentation and Certification of Corrective Action for citation 1 item 2 issued on August 31, 2023, with an abatement due date of September 1, 2023.

29 CFR  1926.453(b)(2)(v):A body belt with lanyard attached to the boom or basket was not appropriately worn by employee(s) working from an aerial lift.  On or about May 31, 2023, an employee engaged in roofing activities was working from an aerial lift without the use of a body belt or harness attached the basket.  The employee was exposed to a fall hazard.

29 CFR  1926.454(c)(3):When inadequacies in an affected employee's knowledge of use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill, the employer shall retrain the affected employee.  On or about May 31, 2023,  an employee working from an aerial lift was not protected from fall hazards and demonstrated inadequacies in their knowledge of fall protection systems.
