On or about May 30, 2023, employees handling corrosive chemicals but not limited to Krylon industrial Acryli-quik in the metal fabrication area, were exposed to eye injury due to the eye wash bottles being expired.

Employers must use natural and synthetic fiber-rope slings that have permanently affixed and legible identification markings stating the rated capacity for the type(s) of hitch(es) used and the angle upon which it is based, type of fiber material, and the number of legs if more than one. On or about May 30, 2023, employees engaged in the metal fabrication used fiber-rope sling in the back of shop wrapped around banana support structure, employees were exposed to struck by hazards due to be using a sling that did not have the required marking and was frayed.

29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. On or about May 30, 2023, employees engaged in the metal fabrication were exposed to safety hazards due to not have the proper machine guarding in following instances: a.) the back of press machine is unguard exposing employees to rotating parts while removing sheets of metal. b.) the point of operation in front of rolling machine expose employees to pinch points while bending sheets of metal.
