29 CFR 1910.28(b)(13)(ii): The employer did not ensure that when work is performed at least 6 feet (1.6 m) but less than 15 feet (4.6 m) from the roof edge, each employee is protected from falling by using a guardrail system, safety net system, travel restraint system, or personal fall arrest system. On or about June 01, 2023, during the repair of a rooftop HVAC unit on a commercial building located at 201 Granger Road, Brooklyn Heights, OH 44131, The employer did not ensure an employee working on the roof of a commercial building was protected from a fall of approximately 22.2 feet by the use of a guardrail system, safety net system, personal fall arrest system and/or combination warning line system and safety monitoring system.
