Green St., Clinton, Missouri- Employees were observed working and walking underneath elevated employees, materials and equipment and were not protected from impact by the use of hard hats.

29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  Worksite located at 123 Green St., Clinton, Missouri- Employees were exposed to fall hazards and were observed working on low sloped roof as well as a steep roof without the use of any means of fall protection.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:   Worksite located at 123 E. Green St., Clinton, Missouri- Employees were exposed to fall hazards in that the ladder being used to access and egress off the roof was not extended at least 3 feet above the upper landing surface.
