The employer did not establish and implement an accident prevention program where employees were exposed to hazards.

Employees were observed installing materials and working at height.  Employees were exposed to a fall distance of greater than 15 feet eave to ground without use of fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  At the residential construction site located at 212 NW 107th St. in Kansas City, Missouri: Employees were exposed to fall hazards. Employees were observed accessing and egressing the roof by way of portable ladders that did not extend 3 feet past landing.  Employees were exposed to a fall distance of greater than 12 feet eave to ground.
