29 CFR  1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a) New residential construction located at 6615 Benchwood Path, Eastwood Homes, Lot 2 Cumming, GA 30041: On or about 6/5/2023, the employer exposed employees to struck-by hazards while operating pneumatic nail guns, in that protective eyewear was not provided while operating these tools engaged in siding activities.

29 CFR  1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   a) New residential construction located at 6615 Benchwood Path, Eastwood Homes, Lot 2 Cumming, GA 30041: Pump jack scaffold: On or about 06/05/2023, the employer exposed employees to fall hazards of 19-feet, 9-inches to the lower ground level, in that a fall protection system was not provided to the employee working on a scaffold platform engaged in siding activities.
