29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) 236 S Jackson, Corner of Almada and Jackson, Denver CO: On or about June 6, 2023, and at times prior, employees were exposed to fall hazards while conducting framing operations on a three story residential construction site.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  a) 236 S Jackson, Corner of Almada and Jackson, Denver CO: On or about June 6, 2023, and at times prior, employees were exposed to fall hazards while using an extendable ladder that was not properly secured to the above level.
