OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.  a) On or about 6/2/2023, at site located at 1395 Troutman Blvd NE, Palm Bay, FL. in the parking lot, employees were exposed to caught in between hazards when operating the SCAG V-Ride II (Model No.: ECV740 SVR52V-25CV-EFI) mower while simultaneously operating the Shindaiwa Blower (Model No.: EB77ORT).

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a) At site located at 1395 Troutman Blvd NE, Palm Bay, FL. 32905: On or about 06/02/2023 Top Notch Lawn Care, LLC failed to report to OSHA within 24-hours a work-related amputation of an employee. The employer notified OSHA on 06/05/2023 of the amputation.
