At 1250 3rd Street North, Jacksonville Beach, FL: On or about 6/8/2023, the employer exposed employees, performing car repairs using the automotive lift in bay three, to crushing hazards in that the lift was leaning.

At 1250 3rd Street North, Jacksonville Beach, FL: On or about 6/8/2023, and at times prior thereto the employer exposed employees to electrical hazards in that employees were allowed to operate breakers mounted in a cabinet with the front cover removed and was not dead front exposing live unguarded 120/220 volts AC electrical parts.

Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, were not legibly marked to indicate its purpose, unless located and arranged so the purpose is evident:  a. At 1250 3rd Street North, Jacksonville Beach, FL: On or about 6/8/2023, on the west wall of the shop, the overcurrent devices in the panel power supply were not identified, exposing employees to fire  and caught-in hazards.

At 1250 3rd Street North, Jacksonville Beach, FL: On or about 6/8/2023, on the west wall of the shop, unused openings in the panel power supply to the facility were not effectively closed,  exposing employees to a 220-Volt electrical shock hazard.
