29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed  LOCATION: The North side of the jobsite, between the new construction and the existing building at 2 Stillman Place.  On or about June 7th, 2023 the employees of Unitedwor VM Contractor Corp. were exposed to falls and serious injury when a portable ladders was used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  LOCATION: The East side of the jobsite; rooftop, of the new construction at 2 Stillman Place.  On or about June 7th, 2023 the employees of Unitedwor VM Contractor Corp. were exposed to falls and serious injury when each employee on a walking/working surface with an unprotected side or edge which was five (5) stories above the street below while working where fall protection had not been used or provided.
