29 CFR  1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.   a) 550 Glen Ave, Moorestown, NJ: Employees were observed working near skylights that were not protected against fall hazards. An employee fell through the unprotected  skylight twenty three feet to the floor below. Observed on or about 06/12/2023  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function:  a) 550 Glen Ave, Moorestown, NJ: Employees were observed working on a flat roof, at a height of approximately 30 feet, without a safety monitor in place. The employer erected a rope barrier with 14 employees working but the safety monitor performed other duties on the ground. Observed on or about 06/12/2023  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
