29 CFR 1926.20(b)(2): The employer did not provide for frequent and regular inspections of the job sites, materials, and equipment or ensure inspections were made by competent persons designated by the employer:   (a)Tovar Sandoval Construction LLC at 800 North Ebenezer Avenue, Sioux Falls, SD 57107:  On or about and at times prior to June 12, 2023, the exposing employer did not ensure an accident prevention program was developed and implemented for the work environment to include regular inspections of the jobsite, materials, and equipment; conducted by a competent person. Employees were exposed to hazards while working with equipment, to include but not limited to, scaffolding systems on site without appropriate training on the process or procedures. This condition exposed employees to fall and struck-by hazards.
